Measurable annual goals

An explanation of the extent to which the student will or will not participate in general education programs.
Projected initiation date, duration, and frequency of services. In addition to location of support services.

What is a 504 Plan?

A written document developed by a LBUSD team that qualifies a student with mental or physical impairment which substantially limits one or more of a person's major life activities.
